Clinical Documentation and Procedure Based Treatment Planning

Scope of Course:
This course offers an in-depth review of proper documentation protocols from the physician's order through evaluation, treatment and discharge. Medicare coverage criteria will be reviewed as well as special coverage issues. General guidelines for documentation will be examined as well as discipline specific considerations for skilled documentation and billing to support claims. Treatment planning based on Current Procedureal Terminology (CPT) and International Classificaiton of Diseases - 9th Edition (ICD-9) correct coding will be explored. Therapists will identify how to select and accurately record appropriate codes based upon the code relevance to the documented plan of care. Using multiple examples, therapists will learn how to be compliant with coding and how to avoid common errors. Attendees will participate in peer review documentation and audit process to guage learning. Target audience includes PT, PTA, OT, COTA and SLP.
Learning Objectives: Upon completion of this seminar, participants will be
able to:
1. Describe when Rehabilitaiton Services meet Medicare coverage criteria
2. Demonstrate skill set and knowledge of their professional discipline through clinical documentation
3. Identify at least three special coverage issues
4. Discuss how clinical documentation creates the clinical picture of the patient's overall condition and response to treatment
5. Discuss at least four specific requirements for Rehabilitation documentation
6. State at least two Rehabilitation billing requirements for Medicare Part B patients
